A C-peptide test in India costs roughly ₹400 to ₹1,800 (about $5 to $22 USD) at a commercial diagnostic lab — Dr. Lal PathLabs, Metropolis, Thyrocare, and SRL cluster in the ₹600–₹1,200 band for a single fasting serum C-peptide by chemiluminescent immunoassay, with Thyrocare typically the floor near ₹400 and hospital-attached labs in Mumbai or Delhi reaching ₹1,800–₹2,500. That is 10x to 40x cheaper than the $95–$250 a US reference lab bills for the same CLIA assay. The Two Things People Mean By "C-Peptide Cost"

The query splits into two non-overlapping markets, and conflating them wastes procurement cycles.

Clinical diagnostic testing. A blood draw analyzed by an accredited lab. Priced per sample. In India, NABL-accredited chains publish list prices openly:

  • Thyrocare C-peptide (fasting, serum): approximately ₹400–₹550

  • Metropolis Healthcare: approximately ₹1,000–₹1,400
  • SRL Diagnostics: approximately ₹950–₹1,300

Reference material procurement. A characterized, documented peptide sold by mass to a laboratory. Priced per milligram, with analytical documentation attached. Human C-peptide is a 31-residue chain (EAEDLQVGQVELGGGPGAGSLQPLALEGSLQ, molecular weight ≈ 3,020 Da) and it is synthesized, not extracted. Global catalog pricing for research-grade material sits roughly at $180–$450 per 1 mg at ≥95% HPLC purity, and $600–$1,400 per 1 mg for a certified reference material with a mass-balance-assigned content value and an ISO 17034 producer certificate. Indian domestic synthesis houses quote lower — commonly ₹12,000–₹35,000 per mg ($145–$420) — but the spread reflects documentation depth, not just labor arbitrage.

The price gap between a ₹500 diagnostic test and a $1,200 certified reference vial is not a markup. It is the cost of the paperwork that makes a number defensible.

What Drives Reference-Material Price Per Milligram

Four variables account for nearly all of the variance in a quote.

  • Stated purity threshold. ≥95% area-under-curve by RP-HPLC is the commodity grade. ≥98% typically adds 40–70% to the price because it requires an additional preparative purification pass and discards yield. Ask which detection wavelength the purity was integrated at — 214 nm (amide bond) reports differently than 280 nm, and C-peptide has no tryptophan and no tyrosine, so a 280 nm chromatogram is close to meaningless for this sequence. A supplier quoting 280 nm purity on C-peptide does not understand the molecule they are selling.
  • Identity confirmation. ESI-MS or MALDI-TOF confirming the observed monoisotopic mass against the theoretical value. A batch-specific spectrum in the CoA, not a generic one, is the standard. See our quality documentation for what our own CoA packets contain.
  • Content assignment. Net peptide content by amino acid analysis or quantitative NMR, distinct from chromatographic purity. A vial labeled "1 mg" at 95% HPLC purity may contain 700–800 µg of actual peptide once counterion (typically TFA, 5–15% by mass) and residual water are subtracted. This single distinction is the most common source of a 20–30% quantitation error in downstream work.
  • Batch size and lot traceability. A 5 mg order priced at $1,400 works out to $280/mg; a 100 mg order from the same synthesis lot often lands at $60–$90/mg. Ordering across a single lot also removes lot-to-lot variance from a study, which matters more than the unit price for anyone running a method over 6–18 months.

Shipping, Customs, and the Total Landed Cost from India

A quoted ₹18,000/mg from a Hyderabad or Bengaluru synthesis house is not the number you pay. Build the real figure:

  • Cold-chain courier, India to US, 1–3 kg dry-ice or gel-pack shipment: $180–$450
  • Customs brokerage and entry filing: $75–$200
  • US duty on peptides under HTS 2937/3822 classifications: frequently 0%, but misclassification triggers 3–6.5% plus a correction delay
  • Transit time: 4–9 business days typical, 3–5 weeks when documentation is queried
  • Failed-shipment rate on first-time cross-border cold-chain: realistically 5–10%, usually paperwork rather than temperature excursion

On a 5 mg order at ₹18,000/mg (₹90,000 ≈ $1,080), landed cost climbs to roughly $1,400–$1,700 — a 30–55% premium over the quoted price. When This Is Not Worth It — Read This Before You Buy

This section will cost us orders. It is still the right advice.

  • You want to know your own C-peptide level. Buy the ₹500 lab test. Do not buy reference material. A vial of lyophilized peptide standard has no diagnostic function whatsoever and cannot answer a question about a person. Materials sold as research use only are exactly that, and nothing on this site is offered for or capable of any personal use. Stop here.
  • You need results traceable to a national metrology institute. Research-grade material — ours included — is not an ISO 17034 certified reference material. If your method validation, regulatory submission, or proficiency-testing program requires CRM traceability to NIST or an equivalent NMI, buy the CRM at $600–$1,400/mg. Saving $400 on a research-grade vial and then having a reviewer reject the traceability chain costs far more than $400.
  • Your assay is an immunoassay and your antibody drives everything. C-peptide immunoassays vary widely in cross-reactivity with proinsulin and its split products. If your antibody sees proinsulin at 5–20%, a purer calibrator peptide does not fix a specificity problem. Pay for antibody characterization first; the standard is the cheaper variable.
  • You will use under 0.5 mg over two years. Buying 5 mg to get a better per-mg rate means 4.5 mg sitting in a freezer past its supported window. At −20 °C, lyophilized C-peptide is typically supported for 24–36 months; in solution at 4 °C, days. You will discard most of what you bought, and the effective per-mg cost of consumed material exceeds the small-quantity price you were avoiding.
  • You cannot maintain −20 °C or colder without interruption. Freeze-thaw cycling and ambient humidity ingress into a hygroscopic lyophilizate are the two most common causes of a standard curve drifting between preparation dates. A supplier CoA becomes irrelevant the moment storage discipline fails, and the failure looks like a bad batch rather than a bad freezer.
  • Your budget cannot absorb a rejected lot. Plan for a 5–15% rate of material that fails incoming QC on a new supplier. If a single rejected shipment breaks the project, qualify the supplier with a small paid order before committing the main purchase.

Failure Modes Specific to Cross-Border Sourcing

  • CoA without a lot number — or with a lot number that does not match the vial label. Roughly one in four low-cost overseas quotes we have seen arrives with a template CoA. It is not evidence of anything.
  • Purity by a single method only. HPLC purity without a mass spectrum tells you the material is homogeneous, not that it is C-peptide. Deletion sequences co-elute more often than suppliers admit.
  • Salt form unstated. TFA vs. acetate vs. free base changes mass-to-content ratio by 5–15%. If the CoA does not state it, your concentrations are wrong by an unknown margin.
  • Reshipped material. Small distributors buying bulk and re-vialing under their own label break the traceability chain to the original synthesis. Ask who synthesized the lot, not who sold it.
  • Currency and duty timing. A quote in INR held for 30 days can move 1–3% on FX alone before the invoice settles.

A Realistic Procurement Budget

For a lab standing up a C-peptide method from zero, plan roughly:

  • Qualification purchase, 1 mg research grade with full CoA: $180–$450
  • Confirmatory in-house identity check (MS time, analyst hours): $200–$600
  • Working stock, 5–10 mg single lot: $600–$2,500
  • One CRM vial for calibration anchoring, if required: $600–$1,400
  • Cold-chain logistics across the year: $200–$900
  • Total first-year material and documentation: roughly $1,800–$5,800

Against that, the ₹400–₹1,800 clinical test price is not a comparable figure and should not appear in the same budget line. Volume buyers should look at wholesale terms, where single-lot pricing at 100 mg scale changes the arithmetic substantially.

If two quotes differ by 60% and one of them is missing three of those seven items, they are not the same product and the cheaper one is not cheaper.
